Bridging Futures™

Program Vision

Our vision for the Bridging Futures™ program is a world where young adults transition into adulthood with stability, community, and the skills they need to thrive.

Approach

Our team approach to supporting young adults transitioning out of foster care:

  • Builds essential life skills for adulthood
  • Strengthens long-term social networks and support systems
  • Walks alongside young adults as they navigate education, employment, personal growth, and daily life.

What We Do:

  • Bridging Futures™ serves young adults ages 17–21 who are in the custody of the Department of Social Services.
  • The program provides individualized support across educational, vocational, social, and independent living domains.
  • Bridging Futures’ teams include professional foster parents, a licensed clinician, a care manager, and a supervisor. Each team serves no more than 15 young adults, allowing for high levels of attention and support.
  • Bridge Parents are professional foster parents and full-time, salaried agency employees. They focus solely on supporting young adults as they navigate early adulthood.
  • All program staff are trained in the Bridging Futures™ core curriculum, Trust-Based Relational Intervention®, the Sanctuary Model©, and Empower Yourself life skills curriculum.
  • The goal of Bridging Futures™ is to address unmet needs and improve the health, well-being, and long-term stability of young adults transitioning out of foster care.
Bridging Futures™ is modeled after our Bridging Families™ program, which has been endorsed by the North Carolina Department of Health and Human Services as the state model for professional foster parenting.
